How do I find out if I was affected by the T-Mobile data breach in 2021?

I was given a T-Mobile phone by a organization for.services provided and my information was compromised because of.the August 2021 breech. I never received notice because I am not the account holder. I have 3 sources which gave t-mobile as the cause. My DL and SS numbers are on the darkweb. What can I do?
