Insurance coverage on a phone

Hey there! So sorry but no one here has access to your account as this is an unsecure open forum- do you have access to MyT-Mobile.com? I can tell you what to look for if so, otherwise you might want to reach out to your Team of Experts at 611 to look over your account.
If you have a T-Mobile ID, go to my.t-mobile.com and sign in. On the main page, you'll see a list of all of the devices on your account- choose the correct device/line. On the next page, you will see Add-Ons. Insurance will be indicated by an entry here that reports either PHP, JUMP!, or Protection<360> (or similar verbiage- your mileage may vary based on your plan and feature).
