Keep and switch reimbursement process

A couple of days back we successfully ported 3 lines from VZ to T Mobile. We were still paying off the 3 phones to Verizon so now I’m trying to redeem the up to $800 rebate offer from T Mobile. I started the process with one number but nowhere in the process am I able to add the other 2 numbers on my plan. My question is do I have to submit the Keep and Switch reimbursement for each number separately? Wouldn’t that be more than one offer redemption per account and rejected? What am I missing? Thanks!
